Benefits of Home Care and Hospice: "Home: Where You Want to Be" is a Wellness Ministry program sponsored in coordination between Advocate Condell Medical Center and Ivanhoe Congregational Church. It will be held on Monday, August 24, at 7 pm at Ivanhoe Congregational Church, 21078 W. Route 176, in Mundelein.

Empower yourself to make informed health care choices! Jodi Ozols from Advocate Good Shepherd Hospital will present the Advocate At Home Program, which includes nursing and hospice care, home medical equipment and infusion, pharmacy services, respiratory and rehabilitative therapies and more.

The Benefits of Home Care and Hospice Program is free and prior registration is not required.
