Elgin starts kindergarten orientations this month

Families whose children plan to attend Elgin Area School District U-46 as kindergartners for the 2012-2013 school year should attend kindergarten orientation this month to preregister for classes. Children who will turn 5 years old on or before Sept. 1, 2012, are eligible to register.
Families are asked to register at the school that their child will be attending. For those who are unsure of which school serves their address, visit www.u-46.org.

Parents or guardians must bring the child's birth certificate and three proofs of residence in the district. Parents can apply to the county clerk in the county where the child was born to receive a copy of the birth certificate. Registration forms, immunization requirements and other materials will be distributed at the orientation.

The Illinois School Code requires preschool, kindergarten, sixth- and ninth-grade students to submit a report of physical examination and the dates that certain immunizations were received in order to attend school. The state also requires a vision exam completed by a licensed eye doctor for all kindergartners and students entering an Illinois school for the first time; and a dental exam for kindergarten, second and sixth grades. A lead screening is required and a Mantoux test for tuberculosis is recommended. Immunity to diphtheria, pertussis, tetanus, polio, measles, mumps, rubella and hepatitis B is required. All immunizations must be recorded on the state approved examination form.
U-46 has a First Day Exclusion policy for physical examinations and immunizations. Physical exam documents must be brought to the school on or before the first day of class.
Local physicians and all U-46 schools have a supply of the state-approved physical exam forms. The forms are also available on the district website, www.u-46.org/healthservices/HealthForms.htm.
