New student registration: July 6 to August 17

Parents of children who will be new students at District 54 schools can register this summer at the District 54 Office, 524 E. Schaumburg Road in Schaumburg. Parents are encouraged to enroll their children as soon as possible after moving into our school district.

Only those children whose fifth birthday falls on or before Sept. 1, 2011, are eligible to enroll in kindergarten for the fall term, 2011. This is a state of Illinois regulation. District 54 does not offer early entrance.

Summer registration hours from July 6 to August 4 are as follows:

10 a.m. to 7 p.m. on Tuesdays and Wednesdays

10 a.m. to 2 p.m. on Thursdays

Summer registration hours from August 4 to 17 are as follows:

10 a.m. to 7 p.m. on Tuesdays and Wednesdays

At registration, parents will be asked to complete the following documents (many of which are available on our website at http://sd54.org/registration):

• Student Enrollment Form

• Family Census Form

• Proof of Residency Form

• Consent for Release of Information Form (this allows our school district to obtain the student's records from the former school district)

• Invoice for Consumable Materials Fee

Consumable materials fees cover the cost of workbooks, paper, art supplies and other items used by students. Our consumable materials fees are $50 for all students from early childhood through junior high school for the 2011-12 school year. A late fee of $15 will be imposed on any payments received after Nov. 15. The late fee will be increased to $25 if no payment is made by Feb. 1 and to $35 if no payment is made by April 1. Children who qualify for the free lunch program will not be charged this fee.

Parents must also provide the following information to complete the enrollment process:

• Student Transfer Form – In Good Standing (for students transferring from one Illinois public school district to another; this form is provided to the parent at the time of withdrawal from previous school)

• Proof of immunizations

• Birth Certificate (original with seal)

• Proof of residence

• Phone numbers of people to be contacted in emergencies (neighbors, other family members and work numbers)

To prove residency, parents must provide

A copy of at least one of the following documents:

• The most recent real estate tax bill for residence

• Signed lease for the residence

• A closing statement for the purchase of my residence

• Form 891 – Letter of residence in lieu of lease (available at time of registration), plus a signed lease, closing statement or tax bill from the owner plus 2 items from the following list from owner and 2 from person residing in home.

In addition, a copy of at least two of the following documents:

• Driver's license

• Gas, electric or telephone bill

• Public Aid card

• Voter registration card

• Home/apartment insurance certificate

• Automobile registration – State of Illinois

• Receipt for city vehicle sticker

Any person asserting legal custody over a student, who is not the student's parent or legal guardian, must complete the Nonparent Custody Form (available at time of registration).
