Information kit for seniors

The Countryside Fire Protection District is making free “My Medical Information” kits available to senior citizens. The kits, funded by Advocate Condell Medical Center, consist of a red plastic pouch with a magnetic back to be attached to a refrigerator. Personal information, emergency contacts, medications, allergies and medical conditions can be written on a cardboard pamphlet inside the pouch. A sticker to be placed on or near your front door, alerts responding paramedics that the pouch is on the refrigerator. Contact Administrative Assistant Lina Diaz (847) 367-5511 from 8 a.m. to 4 p.m. weekdays or email ldiaz@countrysidefire.com and write “My Medical Information Kit” in the subject line. Include your name and mailing address in the body and a kit will be mailed or interested seniors can get one at the fire station, 600 Deerpath Drive, Vernon Hills.
