Carol Stream Dist. 93 to hold COVID-19 vaccine clinics for 12 or older in June
Carol Stream Elementary District 93 is partnering with Jewel-Osco to host a COVID-19 vaccination clinic for individuals age 12 or older on Wednesdays, June 2 and 23, at Jay Stream Middle School, 283 El Paso Lane in Carol Stream. Register at www.ccsd93.com/12andover.
Please bring the following with you to your appointment:
• A completed copy of this consent form.
• If you have health insurance or participate in Medicare, bring your insurance card or Medicare ID
• Your child's birth certificate, driver's license, state issued photo ID.
