The Advocate Children's Hospital Ronald McDonald Care Mobile will be coming to Community Unit District 300 this summer.

The mobile clinic provides easy access to quality health care for students with Medicaid as their primary insurance to assist in obtaining the state of Illinois required school physicals and immunizations.

This program does not accept private insurance or payment.

Contact Sherrie Schmidt at (847) 551-8371 if you have no insurance to see if your student might qualify for these services.

Please call (847) 551-8371 to schedule an appointment for one of the clinics, which will be offered from 9:30 a.m. to 3:30 p.m.

• Wednesday, July 11, at Eastview Elementary School, 540 Longwood Drive, Algonquin;

• Wednesday, July 18, at Carpentersville Middle School;

• Wednesday, Aug. 8, at Dundee Crown High School.

To provide the best and safest care for your student, the following forms need to be completed: patient information (demographic) sheet which includes consent for treatment; child history form and/or sports history form; vaccine (shot) records (if not provided to the school); and insurance information (Medicaid plan name and ID number).

Completed forms must be returned at least one week prior to scheduled appointment. Forms can be scanned to Health.Forms@d300.org or faxed to (847) 551-8433 or dropped off to the District 300 Central Office.

The parent or guardian's consent for treatment will allow the clinic to immunize the student according to Centers of Disease Control and Illinois Department of Public Health guidelines. For information on the vaccines, visit www.cdc.gov/vaccines/hcp/vis/current-vis.html

Parents/guardians are encouraged to participate in the student visit, but if they cannot attend, include a phone number where they may reached during the clinic hours.
