The Chicago office of the FBI has created an online questionnaire to help identify potential victims of a Hoffman Estates doctor who recently was indicted on 13 counts of heath care fraud.

In court papers, Dr. Mona Ghosh, 50, is accused of knowingly submitting false reimbursement claims totaling about $796,000 for treatment that was never delivered between February 2018 and April 2022.

Ghosh, an Inverness resident, owned and operated Progressive Women's Healthcare in Hoffman Estates during the time the false claims were submitted, according to court records.

The FBI asks potential victims to visit www.fbi.gov/MonaGhoshInvestigation. Victims may be eligible for certain services and restitution.
