Ill. vets exposed to incorrect drug doses

WASHINGTON -- Veterans Affairs health centers nationwide, including in Illinois, suffered software glitches that showed faulty displays of electronic health records.

Documents obtained by The Associated Press indicate the glitches began in August and lingered until December.

The VA Illiana Healthcare System in Danville, Ill. is among the centers where data mix-ups occurred.

The problems weren't disclosed to patients even though they sometimes involved prolonged infusions of drugs.

There's no evidence any patient was harmed.

But the issue's more pressing with a federal push for universal use of electronic medical records to avert mistakes often attributed to paper systems. Experts say the VA's problems illustrate the need for close monitoring.
