MH 12 LEGACY SCHOLARSHIP
DEADLINE FOR SUBMISSION APRIL 4 TO SCHOOL COUNSELORS
Grayslake, IL--12 Oaks Foundation will award The MH12 Legacy Scholarship in the amount of $1,500 to one graduating senior from each of the following high schools - Grayslake Central, Grayslake North, and Carmel Catholic.
The students selected for this scholarship will have demonstrated, and be evaluated on, leadership, honesty, integrity, resilience, and teamwork through sports and/or volunteer work.
The scholarship honors Matt Hupp of Grayslake, who lost his life to cancer in 2009 at the age of 13. The life lessons Matt learned as an athlete helped him in his fight with cancer and have inspired countless others to dedicate themselves to excellence both on and off the field.
"This scholarship allowed me to reflect on my involvement in athletics. I realized that through sports, I became the person I am today and learned more about the person I strive to be--a person who changes lives, specifically, a nurse. My decision to pursue a nursing career at the University of Wisconsin-Madison stems from the many leadership opportunities that high school athletics presented me. I want to live my life serving others and impacting those around me, just as Matt's leadership impacted me. Matt was truly an inspiration to so many, and his legacy will live on with me forever."

SPECIFIC CRITERIA -
• Participation in a minimum of one complete season in high school sports program
• Demonstrated leadership and strong character traits gained in or demonstrated through sports and/or community service, work, and academics
• Class Rank of at least top 50% or minimum 2.5 GPA (4.0 scale)
• Senior with proof of acceptance or intent to attend a 2 or 4-year Institution
