Greater Family Health opens 12th health center in Des Plaines

Greater Family Health, the fifth largest Community Health Center in the state of Illinois, opened its 12th Community Health Center, located in Des Plaines, on Aug. 18.

The new facility located at 1515 Lee St., is made up of 12 state-of-the-art examination rooms, a full-service LabCorp laboratory, and a CLIA-waived lab for point-of-care testing.

“We are honored to expand our services to the residents of Des Plaines and surrounding areas,” said Lupe Fonseca, president and CEO of Greater Family Health. “This new health center represents our commitment in sharing our mission to provide quality and affordable healthcare to all, regardless of their ability to pay. We look forward to building strong, healthy, and lasting relationships with our new neighbors.”

The new health center in Des Plaines will initially offer family medicine, pediatrics, obstetrics and gynecology services.

Care will be provided by four practitioners including two experienced Greater Family Health Doctors, Dr. Anh Si Le, D.O., and Dr. Cheerag Patel, D.O.

Joining them will be two new providers, Alicia Schanbacher, MSN, APN, FNP-C, a true professional with over 20 years of nursing experience, and Fabiola Martinez, MSN, FNP-C, a dedicated Greater Family Health employee since 2014 who has advanced through multiple roles in the organization. All four are highly talented practitioners and will be a valuable asset to the area’s residents.

This new health center will be in good company, joining a network of 11 Community Health Centers, providing a comprehensive range of high-quality, affordably priced, fully integrated primary care services to over 76,000 area residents via 246,000 medical, behavioral, and oral health appointments annually across five counties in Northeastern Illinois.

All centers are accredited by The Joint Commission, are Primary Care Medical Home certified and nationally recognized for both health outcomes and cost efficiency. Greater Family Health provides care to all, regardless of the patient’s ability to pay.
