New Carepods clinics in Arlington Heights and Oak Brook improve health care access for older adults
Community Physicians, a Chicago-based physician group specializing in health care for older adults, announced today the launch of a multi-location, primary care practice called Carepods which is dedicated to meeting the health care needs of older adults. The primary care clinics are located in Arlington Heights and Oak Brook. They are the first of their kind to exclusively serve older adults with a proprietary, extended care service model.
The Carepods operating model is delivered using a physician spoke model, or a “Pod” where physicians lead assigned care teams who support patients during and in-between visits to promote better health outcomes. This increases the amount of time a physician can spend engaging with their patient, improving the experience and quality of care they receive.
“In a typical primary care setting, physicians may see each patient for a few minutes only and then due to the high number of appointments that day, struggle to follow up,” said Dr. Fatima Hyder, medical director of Carepods. “With our approach, patients are surrounded by an extended care team who triages requests, answers questions and ensures quality time spent with their physician during appointments.”
The Carepods primary care teams also help patients access in-house specialist appointments, follow up on physician orders, set appointments and ensure the older adult has a resource to help them better navigate and access care. The teams develop long-standing relationships with their patients and families, often following their progress and stepping in to assist as care transitions occur.
Hyder says the opportunity to elevate health care for older adults in Arlington Heights and Oak Brook is substantial.
