How to vote if you're hospitalized or moving into a nursing home

Registered voters in Lake County who are admitted to a hospital, nursing home, or rehabilitation center within 14 days of the April 6 election may qualify for personal delivery of a ballot by a family member or friend, subject to statutory conditions.

Voters must complete a request form titled "Application for Ballot for Qualified Voter Admitted to a Hospital, Nursing Home, or Rehabilitation Center" and a physician is required to complete the accompanying affidavit.

All required forms must be completed and returned to the Lake County clerk's office prior to the ballot being issued. Forms are available by contacting the clerk's Election Department at VotingByMail@lakecountyil.gov or (847) 377-VOTE.

Or, voters can download the hospitalized voter form by visiting LakeCountyClerk.info, clicking on the "April 2021 Election" icon, and following the link to "Hospitalized Voter."

Voting by mail also is an option. The required electronic or paper ballot application must be received at the clerk's office by 5 p.m. Thursday, April 1, and a completed ballot must be hand delivered to the clerk's office by 7 p.m. on Election Day, or it may be returned via mail with a postmark no later than April 6 and received in our office by April 20. A request for a mail ballot can be made by logging on to LakeVoterPower.info and completing the online application, emailing your name, address, and birth date to VotingByMail@lakecountyil.gov, or by calling (847) 377-VOTE.
