Hampshire woman killed in Burlington Twp. crash early Sunday

A 57-year-old Hampshire woman was killed early Sunday when the car she was driving was rear-ended and forced into a utility police near Plank and Romke roads in unincorporated Burlington Township, authorities said.

Cindy Formas was pronounced dead at the scene of the two-vehicle crash, which was reported about 1 a.m. to Kane County Sheriff's police.

According to the sheriff's office, Formas was driving a Ford Fusion west on Plank Road when for an unknown reason a Kia Optima also heading west struck her car from behind, causing the Ford to hit the pole.

The driver of the Kia, Kyle Luebke, 28, of DeKalb, was taken to a hospital in Elgin for treatment of nonlife-threatening injuries, sheriff's police said. Both drivers were the only people in their cars.

Sheriff's deputies and the Kane County Accident Reconstruction Team continue to investigate the cause of the crash. No citations have been issued, and detectives are still trying to determine if drugs, alcohol, texting or excessive speed were involved in the crash, authorities said.
