Voting while in hospital, nursing home

Registered voters who are admitted to a hospital, nursing home, or rehabilitation center within 14 days before the March 15 primary election may qualify for personal delivery of a ballot, subject to certain conditions. The voter must complete a form and the voter's physician is required to complete an accompanying affidavit. Forms are available by contacting votingbymail@lakecountyil.gov or (847) 377-2314 or by visiting LakeCountyClerk.info. The form must be returned to the Lake County Clerk's office.
