Lake County health survey:

The Lake County Mobilizing for Action through Planning and Partnerships team has launched a survey to identify community health strengths and challenges. Residents and individuals who work in Lake County are invited to take the 10-minute survey available in English or Spanish. Responses are anonymous. Complete the survey by Friday, Oct. 21. Also, encourage friends or colleagues (who live or work in Lake County) to also complete the survey. MAPP is a community partnership of local individuals and groups, including the Lake County Health Department, working to improve the quality of life in Lake County. The survey will assist the MAPP team to gather information and develop a broad-based community health plan. The survey results will be available in November at: http://www.lakecountyil.gov/Health/Pages/MAPP%20Project.aspx.
