Who really pays for cigarette tax?

The smoker doesn't suffer by the added taxes. They will pay whatever the pice for their nicotine addiction. It's the family of the smoker(s) that suffer.

That money could be much better spent on a family outing - a dinner out, a weekend away, an occassional movie. Tax away, it's not affecting those you think it is.
