Local doctor remembered for his skill, compassion

Dr. John Kozak, who was one of the first urologists to treat patients at Northwest Community Hospital in Arlington Heights and at Alexian Brothers Medical Center in Elk Grove Village, has passed away.

As the founder of Northwest Urological Associates in the mid-1960s, in Arlington Heights, colleagues say he was the first urology specialist to open in the Northwest suburbs.

He passed away on Saturday. The longtime Elk Grove Village resident was 78.

A framed photo of Dr. Kozak hangs on the wall on the first floor of Alexian Brothers Medical Center, across from the administrative wing, honoring his service and leadership among his peers.

Dr. Kozak was president of the medical staff there from 1973 to 1974, and in 1974, he was named "Doctor of the Year," by his medical colleagues.

Dr. Charles Pastika, an anesthesiologist and longtime friend of Dr. Kozak, said the award recognized his compassionate nature with patients.

"He was very caring with his patients, and that's what made him successful," Pastika says.

After opening his general urology practice, Dr. Kozak found a large demand for his specialty. In 1970, he added Dr. Thomas Sammons to the practice, before later adding Dr. Daniel Piazza and Dr. David Nellessen.

The latter two doctors still run the Arlington Heights practice, along with two more physicians.

"When he came here, he was one of the few general urologists in the area," Nellessen says. "He was thoughtful and caring, and his patients absolutely loved him. They still ask about him."

Dr. Kozak also was a frequent speaker on health and wellness issues, before community groups, through the Elk Grove Chamber of Commerce, and the Elks Club, colleagues say.

He is survived by his wife of 48 years, Rosemary, as well two children, Catherine (Robert) Koch and Thomas (Angela).

A funeral Mass will take place at 10 a.m. today, Wednesday, at Queen of the Rosary Church, 680 W. Elk Grove Blvd., in Elk Grove Village.
