A federal report shows inconsistencies with reporting deaths at a southern Illinois Veterans Affairs hospital. The report comes more than two years after the facility had at least nine deaths attributed to substandard care.
The report, which covers the fiscal year ending in 2009, was released Monday by the Veterans Affairs Department's inspector general. It says the Marion facility had continued problems with mortality assessment, a patient safety program and peer review.
For example, three sets of documents showed three different death totals for April 2009.
The report also reveals outdated staff training and patient data analysis.
A hospital spokesman didn't immediately have comment. However, included in the report is a memo from the director saying the facility has corrected or is in the process of correcting issues.
