Hoffman Estates offers school physicals in August

The village of Hoffman Estates Department of Health and Human Services will offer back-to-school physicals for school-age children at the Children's Immunization Clinics from 3:30 to 6 p.m. on Mondays, Aug. 4 and Aug. 18, at the village hall, 1900 Hassell Road.

Physicals are available for children attending preschool to grade 12 and will be performed by physicians from Alexian Brothers Medical Group. Physicals are required for preschool, kindergarten, sixth and ninth grades as well as children who are new to the school district. Athletes must obtain a physical every year. High school students need a physical and updated immunization before the first day of class.

The cost for a clinic physical is $10 for uninsured and AllKids, and $30 for those who are insured. Only cash or checks will be accepted.

If your child has not received the necessary shots for starting school, please plan to attend one of these special clinics. Immunization records are required. The cost of vaccines is $8 for residents of Hoffman Estates and Schaumburg, and $10 for all other communities.

Appointments for these clinics are recommended but walk-ins will be accepted. Call (847) 781-4850 to schedule an appointment or to get more information.

Hoffman Estates Children's Immunization Clinics are normally held the first Monday of each month from 3:30 to 6 p.m. The next clinics will be held on Sept. 8, Oct. 6, Nov. 3, and Dec. 1 from 3:30 p.m. 6 p.m. Baby Clinics for children ages two months to kindergarten are for residents of Hoffman Estates only, and appointments are required.
