Several boxes from the lift gate then dropped onto him, Nebl said. Terrell was taken to Alexian Brothers Medical Center in Elk Grove Village, where he was pronounced dead at about noon.

According to the results of an autopsy Monday by the Cook County medical examiner's office, Terrell died from multiple injuries caused by the fall and being crushed by the falling boxes. His death was ruled accidental.
