Case Study: Minimally Invasive Ultrasound Technology Treats Varicose Veins

After undergoing varicose vein surgery 30 years ago, Mary Ann Mayer of Des Plaines was reluctant to see another vascular surgeon. Her surgery was performed in a hospital and required incisions at her ankle, at her knee, and in her upper thigh. She experienced leg numbness afterwards, due to branches of her nerves that were affected by the surgical procedure. She took a full year to recover from the surgery.

In the early 2000's, Mayer's internist referred her to Dr. Susanne K. Woloson, M.D., PhD., RPVI, a partner and vascular surgeon at Northwest Vascular and Vein Specialists (NVVS) which as offices in Arlington Heights and Libertyville. Her doctor made the recommendation after Mayer had suffered a stroke to assess her risk for another stroke and provide comprehensive vascular care, including evaluation of her worsening veins as part of an overall treatment plan.

During Mayer's initial visit, Dr. Woloson performed a comprehensive vascular system check and found a carotid bruit in the woman's neck, which indicated there was turbulent blood flow in her artery. With this finding, Mayer has been continually monitored to this day to ensure there are no changes that could adversely affect her health.

This type of thorough and detailed care is one of the many benefits Mayer has experienced during ongoing treatments performed by Dr. Woloson, and her practice partner Dr. Elizabeth Clark, M.D., R.V.T., F.A.C.S. "I wasn't sure what to expect, because my first procedure was so painful," Mayer said. "I had no idea technology had come so far."

Over the last several years, Mayer's vein problems have been managed with minimally invasive, office-based treatments which have eliminated painful and unsightly varicose veins on both legs, with endovenous laser therapy (EVLT) and ultrasound guided injection Sclerotherapy.

This special type of sclerotherapy treats painful varicose veins that are located under the surface of the legs; these dilated abnormal veins are branches of the EVLT-treated veins. Exact location and precise treatment is made possible with ultrasound imaging performed by Ken Langworthy, the Registered Vascular Technologist at NVVS who works side-by-side with both doctors. This advanced imaging technology allows targeted veins to be identified for accurate treatment.

Sclerotherapy permanently closes the diseased varicose veins with no incisions, and the entire procedure typically takes about half an hour with local anesthesia. Within several weeks, the body is able to absorb and recycle the scar tissue.

Immediately after each of her procedures at NVVS, Mayer was encouraged to walk and resume normal activities immediately. NVVS recommends that patients wear compression stockings for a short period after a procedure is completed, to minimize bruising and swelling and aid in the overall healing process.

Mayer says she feels physically better, and is encouraged by the improved appearance of her legs. As with the overwhelming majority of varicose vein patients, Mayer's procedures were covered by Medicare.

"The ability to treat painful varicose veins with lasers and ultrasound-guided injection sclerotherapy has increased office-based treatment options, and is part of the comprehensive care we offer to our patients," Dr. Clark said. "These treatments are convenient, and allow our patients to get back to their busy lives."

NVVS employs a staff of experts who work directly with patients and their insurance companies. Most insurance carriers provide coverage after an initial trial period of conservative management that includes daily wear of prescription-grade compression stockings, elevation, and regular exercise. All procedures and treatments are pre-certified by NVVS staff before scheduling, to ensure there are no hidden surprises.
