Commercial Ave., Chicago; North Park Village Administration Building, 5801 N. Pulaski, Chicago; and Forest View Fire Department, 7010 West 46th St., Forest View. Individuals may also apply by calling (800) 621-FEMA (3362) or (800) 462-7585 for hearing and speech impaired. Registration also can be done online at disasterassistance.gov or by smartphone at m.fema.org.
Assistance may take the form of grants or low-interest Small Business Administration loans. Funds may be used to help with temporary housing, home repairs or replacement and other disaster-related expenses.
