New Illinois birth certificate rules start Nov. 15

SPRINGFIELD — Adults who were surrendered or adopted soon will be able to request their original Illinois birth certificate.

Starting Nov. 15 those born after Jan. 1, 1946, in Illinois can see their original, noncertified birth certificate. Those versions typically will include birthparents’ names, ages and places of birth. The change comes under the amended Illinois Adoption Act.

The Illinois Department of Public Health is providing a Birth Parent Preference Form for those who wish their identifying information remain confidential. Birthparents who want their information released should fill out a form before Nov. 15.

Birthparents who do want to share medical information can do so through the Illinois Adoption Registry and Medical Information Exchange.
