Lake County Head Start taking applications

The Community Action Partnership of Lake County’s Head Start program is accepting applications for the 2011-12 school year.

Children who will be 3 or 4 years old by Sept. 1 or 5 years old after May 11 may be eligible to participate in the Free Early Childhood Education Program.

Required documents include: a source of income for the past 12 months; immunization record; birth certificate; three emergency contacts (name, address and phone number); and medical card or private insurance care, if applicable.

Also needed are a valid physical exam, dated six months before date of enrollment, as well as results of a dental exam, and tuberculosis, hemoglobin and lead tests. Call (847) 249-4330 for more information.
