District 15 kindergarten registration is Feb. 6

District 15's annual kindergarten and new student registration event is quickly approaching. If you have a new student who will be attending District 15 in the fall of 2018 or an incoming kindergarten student for the 2018-19 school year, be sure to attend Tuesday, Feb. 6, at your home elementary school.

Parents can select from three time slots on Feb. 6 to register their child for the 2018-19 school year: 9:30-11 a.m., 1-3 p.m. or 6-8 p.m.

New families who need to be screened for English language proficiency can also register at Winston Campus Elementary and Willow Bend School Wednesday, Jan. 31, from 5-7:30 p.m.

Incoming kindergarten students must be 5 years old on or before Sept. 1, 2018.

Kindergarten enrollment requires the following:

• Birth certificate (original with seal) or other government document verifying date of birth;

• Physician's telephone number and address, if known;

• Two local emergency contact names and phone numbers;

• Two proofs of residency (i.e., copies of driver's license, lease, or utility bills);

• Any pertinent health information, including health, dental, and eye exams at regular intervals (see www.ccsd15.net | Our Services | Health Services);

• A student transfer form from the previous school attended (if applicable);

• Your child (only if needed to screen for English language proficiency).
