Transitional Care Management Names Michelle Stuercke Chief Clinical Officer
Transitional Care Management (TCM), which provides comprehensive consultative and management services that enhance patient satisfaction, reduce cost and improve outcomes in a variety of post-acute settings, welcomes Michelle Stuercke, RN, MSN, DNP, MPH, LNHA, to the position of Chief Clinical Officer.
A longtime healthcare professional, Michelle is responsible for all clinical programs at TCM properties including Transitional Care of Arlington Heights, a premier stand-alone, purpose-built rehabilitation center in the northwest suburbs, as well as transitional care centers currently under development in Mundelein, Lisle and Aurora, Illinois.
A licensed nursing home administrator, Michelle earned a Master in Public Health degree from Roosevelt University and Doctorate in Geriatric Nursing from Rush University. She offers significant program development, mentoring, regulatory and financial management expertise.
Prior to joining Transitional Care Management, Stuercke served as Chief Nursing Officer for Health and Home Management and Chief Clinical and Learning Officer for NuCare where she was responsible for clinical and quality outcomes at 27 facilities in Illinois, Wisconsin and Arizona. She is a resident of Arlington Heights.
