District 15 walk-in registration is July 12

Parents may register their child (who must be 5 years of age on or before Sept. 1, 2017,) for the 2017-18 school year at their neighborhood elementary school from 7:30 a.m.-1 p.m. Wednesday, July 12.

Please note: several schools are closed this summer due to maintenance and renovation projects. Those schools will be accepting walk-in registrations at their temporary host locations. Visit your school's website for more information before your visit.

Enrollment requires the following information:

• An official birth certificate or other document verifying the date of birth.

• A transfer slip from the previous school attended (if applicable).

• Physician's telephone number and address, if known.

• The name(s) and phone number(s) of a reliable local person(s) to call if you are unavailable in an emergency.

• Two proofs of residency (copies of driver's license, lease, or utility bills).

• Any pertinent health information, including health, dental, and eye examinations at required intervals (see www.ccsd15.net | Our Services | Health Services). A physical examination is valid as long as it was conducted on or after Aug. 16, 2016.

• Your child (only if needed to screen for English language proficiency).

Enrollment information will be mailed over the summer to parents of students who attended District 15 schools during the 2016-17 school year. Registration forms should be updated and returned as soon as possible.

The first day of the 2017-18 school year will be Wednesday, Aug. 16, for grades one-eight. Kindergarten and early childhood classes will begin Thursday, Aug. 17.

School supply lists for each school are available from the schools and are posted on each school's page on the district website, http://www.ccsd15.net/pages/CCSD15/Our_Schools.

Students are usually assigned to their neighborhood school, but in special circumstances they may be assigned to schools outside their neighborhoods. Parents who are not sure which school their children will attend may call the district's Joseph M. Kiszka Educational Service Center at (847) 963-3000.
