Northwest Community Healthcare launches health needs survey
Northwest Community Healthcare has launched an online survey to gauge the health needs of residents in the hospital's 13-town service area.
The hospital is inviting residents of Arlington Heights, Barrington, Buffalo Grove, Des Plaines, Elk Grove Village, Hoffman Estates, Lake Zurich, Mount Prospect, Rolling Meadows, Palatine, Prospect Heights, Schaumburg and Wheeling to complete the survey, which should take 10 to 15 minutes.
Questions cover access to health care, chronic disease, family issues, substance abuse and immunizations. Participants are also asked to identify their top five health needs for the community.
The survey is available at nch.org/survey through March 14.
