District 15 schedules registration dates for kindergartners, new students

All District 15 elementary schools will begin registering kindergartners and other new students entering the District for the 2017-18 school year from 9:30-11 a.m., 1-3 p.m., and 6-8 p.m. Tuesday, Feb. 7. In case of a snow day, the makeup date will be Thursday, Feb. 9.

• At John G. Conyers Learning Academy, kindergarten registration for all currently enrolled CLA and ECDEC students will be from 9:30 a.m.-3 p.m. Monday, Jan. 30, and 4:30-7:30 p.m. Tuesday, Jan. 31.

• New second-language families who need to screen for English language proficiency can also register at Winston Campus Elementary and Willow Bend School from 5-7:30 p.m. Wednesday, Feb. 1. In case of a snow day, the makeup date will be Thursday, Feb. 2.

For the 2017-18 school year, kindergarten students must be 5 years old on or before Sept. 1, 2017.

Enrollment requires the following information:

• An official birth certificate or other government document verifying the date of birth.

• A transfer slip from the previous school attended (if applicable).

• Physician's telephone number and address, if known.

• The names and phone numbers of two reliable local people to call if you are unavailable in an emergency.

• Two proofs of residency (copies of driver's license, lease, or utility bills).

• Any pertinent health information, including health, dental, and eye examinations at required intervals. For more information, see www.ccsd15.net | Our Services | Health Services.

• Your child (only if needed to screen for English language proficiency.

Students are usually assigned to their neighborhood school. Parents who are not sure which school their children will attend may call the district's Joseph M. Kiszka Educational Service Center at (847) 963-3000.
