Hoffman Estates back-to-school physicals

There are still appointments available for school-age children to receive physicals at the village of Hoffman Estates Department of Health and Human Services Back-to-School Physical Clinic on Monday, Aug. 3.

Physicals are by appointment only. Call Health and Human Services at at (847) 781-4850 to schedule an appointment between 3:30 and 6 p.m. at Hoffman Estates village hall, 1900 Hassell Road.

All communities are welcome. The cost is $10 for those who are underinsured, uninsured or have AllKids (cash or check only.)

Required vaccines will be given at the clinic, one hour prior to the child's scheduled physical appointment.

Each vaccine costs $8 for residents of Hoffman Estates, Elk Grove Village and Schaumburg and $10 for all others. Each TB test costs $8 for Hoffman Estates residents and $10 for all others.
